Our Aging Services Founder, Leslee Schafer believes in a holistic and proactive approach that considers one's physical, emotional, and financial needs during any decision making process. With over 15 years of experience, Our Aging Services provides personalized and comprehensive services to help you and your loved ones make informed decisions.
Not all conversations are easy, we understand this is a difficult process. In order to ensure families are equipped with the tools and resources needed to provide the appropriate level of care, we have to ask the hard questions.
With our case management services, we provide guidance and support as your loved one moves through the continuum of care. Our Aging Services offers a range of customized services designed to meet the needs for each person. Each situation is unique and we strive to be as flexible as possible. Care Plan Coordination
If your loved one lives in a long-term care setting, or is in a short term rehab, a care plan meeting should be held with the person's POA and the community team members, including those providing intervention resources to discuss the resident's acuity, deficits, independence, needs, overall successes, and what interventions will be in place going forward.
I am available to assess the resident to determine their needs in addition to coordinating and attending the care plan to provide support in advocating for your loved one's coordination of care. Our support will ensure your loved one is receiving the appropriate services available to them.
